You have lived in the U.S. or its territories for six months or longer

You have no insurance coverage

You are a qualifying Medicare patient with an affordability gap, and you do not qualify for Medicare's Extra-Help Program (Low Income Subsidy)

Patients with insurance plans or employers participating in an alternate funding program (also sometimes referred to as patient advocacy programs, or specialty networks) requiring the patient to apply to a manufacturer's patient assistance program are not eligible for Amgen Safety Net Foundation
